HHCAHPS Survey Scores

Your survey scores are decided between visits.

HHCAHPS asks patients and caregivers what they remember about their care — medications, safety, communication. Visit Bridge reinforces that education automatically, at the exact points in the episode when memory fades, from your agency's own office number.

The Problem

Patients forget. Surveys don't.

Your clinicians teach medication routines, home safety, and self-care at every visit. But a 60-day episode is mostly time without a clinician in the home — and by the time a survey arrives, much of that teaching has faded.

HHCAHPS doesn't measure the care you delivered. It measures what patients and caregivers remember about the care you delivered. Those are two very different numbers.

Visit Bridge closes that gap with automated education and reminders at set touchpoints across the episode — so the people filling out the survey see your agency actively focused on the things that matter to their care, even on days when no visit is scheduled.

What the Patient Sees

One episode. Every touchpoint working for you.

Every message comes from your agency's own office number and opens the same way — "A message from ABC Home Healthcare" — so patients and family members always know exactly who's checking in, and exactly where to reply.

Day 1 · Start of Care
A message from ABC Home Healthcare: Welcome, Margaret. Our goal is simple — to provide the best possible experience and care, the kind we'd want you to recommend to your own loved ones. If you or your family ever have a question, challenge, or concern, reply to this message or call us at this number anytime. We're listening.
Sets the Standard

The bar is set on day one.

From the very first message, the patient and their caregivers hear your standard in plain words: care worth recommending to family. That expectation frames the entire episode.

Day 3 · Medications
A message from ABC Home Healthcare: Taking your medications the right way, at the right time, is one of the most important things you can do for your recovery. Tip: keep a written list of every medication on your refrigerator, set a daily alarm, and review your list with your nurse at each visit. Questions about a medication? Reply here — we'll help.
Reinforces · Medication Education

Medication teaching, remembered.

Your nurses talk through medications at every visit. Day 3 puts that education back in front of the patient — with practical routines — right when the details from Start of Care begin to blur.

Day 10 · Home Safety
A message from ABC Home Healthcare: A safe home is a healing home. This week, take a minute to check for loose rugs, keep walkways clear, and make sure stairs and hallways are well lit — small changes prevent serious falls. Want your nurse to look at anything specific at the next visit? Just reply here.
Reinforces · Home Safety

Safety stays top of mind.

Home safety setup is discussed early and forgotten fast. Day 10 brings it back with tips the patient and family can act on that same day — and an easy way to ask for help.

Mid-Episode · Survey Note
A message from ABC Home Healthcare: At some point, you may receive a short survey by mail or phone asking about your experience with our agency. If one arrives, we'd be grateful if you'd take a few minutes to complete it. Your feedback helps us keep improving the care we provide.
Drives · Survey Response Rate

A gentle heads-up, mid-episode.

Short, warm, and no pressure. The patient simply learns a survey may be coming and that their voice matters — sent once during the 60-day episode.

After Discharge
A message from ABC Home Healthcare: It was an honor caring for you. If a survey about your experience with us arrives by mail or phone, we'd appreciate a few minutes of your time to fill it out. Thank you for letting us be part of your recovery — we're still here at this number if you ever need us.
Drives · Survey Response Rate

Reminded again — right when it counts.

The second reminder lands after discharge, closest to when surveys actually arrive. Not a one-and-done ask: two well-timed touches that lift return rates without a single extra staff phone call.

Done the Right Way

This is not survey coaching.

Patients are never told how to answer, and no message references a specific survey question. Visit Bridge simply reinforces the education your clinicians already provide — at the moments it's most likely to be forgotten.

Never in a message

  • Telling patients how to answer a survey
  • Referencing specific survey questions
  • Scripting or rehearsing responses
  • Tying care quality to survey answers

Always in a message

  • Education your clinicians already teach
  • Practical tips patients can act on today
  • A clear, familiar way to reach your office
  • Your agency's genuine standard of care
Real Agencies, Real Scores

The scores speak for themselves.

These aren't projections. These are published HHCAHPS results from agencies running Visit Bridge touchpoints across every episode.

Wichita Home Health · Wichita, KS
5-Star HHCAHPS — two years running.

Wichita Home Health has used Visit Bridge's automated patient touchpoints to help earn a 5-star HHCAHPS rating in both 2025 and 2026.

Buckeye Home Health · Cincinnati, OH
3 Stars → 4 Stars in one year.

Buckeye Home Health improved its HHCAHPS star rating from 3 stars in 2025 to 4 stars in 2026 with Visit Bridge reinforcing patient education across every episode.

100%
Buckeye Home Health of Dayton · Dayton, OH

A perfect first month on the new survey.

In the very first reporting month of the new HHCAHPS survey released in April, 100% of Buckeye of Dayton's responding patients rated their care satisfaction "very satisfied" or a 9 or 10 out of 10.

Response Rate

More completed surveys. Better data. Fairer scores.

Low return rates let a handful of responses define your agency. Two brief, well-timed reminders — one during the episode, one after discharge — encourage more patients and caregivers to actually send the survey back.

When more of the people you served respond, your scores reflect the care you actually delivered — not just the loudest few.

Sent once · During the 60-day episode
A message from ABC Home Healthcare: You may receive a short survey by mail or phone about your experience with us. If one arrives, we'd be grateful if you'd fill it out — your feedback helps us improve.
Sent once · After discharge
A message from ABC Home Healthcare: Thank you for trusting us with your care. If a survey arrives by mail or phone, a few minutes of your time would mean a great deal to our team.
One Familiar Number

Concerns come to you — not to the survey.

Every message is sent from your agency's own home or office phone number — the same one saved in the patient's phone and posted on the fridge. When a challenge, question, or concern comes up, the patient knows exactly where to raise it: right in the same thread.

Issues get addressed while you can still fix them, in real time — instead of surfacing for the first time as a mailed-in survey answer weeks later.
